Please download and complete these forms if you plan to submit a program for broadcast on Access 12. You should submit the forms at the time you bring in your first episode for playback.
Shows and forms can be delivered to the drop-off area in the Fargo City Commission office (200 3rd St. N.) from 7:45 a.m. to 4:30 p.m., Monday through Friday.
