Sample lease addenum
- The lessee certifies the accuracy of the information provided in connection with the examination or re-examination of the eligibility of the household of the lessee.
- The lessee agrees that the annual income and other eligibility requirements shall be deemed substantial and material obligations of his or her tenancy, and that he or she will comply promptly with all requests for information with respect thereto from the lessor or the government agency.
- The lessee’s failure to provide accurate information regarding income and eligibility requirements (regardless of whether such inaccuracy is intentional or unintentional) or refusal to comply with a request for information thereto shall be deemed a violation of substantial obligation of his or her tenancy and constitute cause for immediate termination thereof.
- The lessee agrees to notify the lessor of any changes in household composition (i.e., if any member of the household moves out of the unit or any new member moves in).
- The lessee agrees to provide the lessor necessary income documentation (i.e., copy of the previous years tax return) each year.
- The lessee agrees to a utility authorization waiver allowing the lessor to obtain utility records for the purpose of determining an average yearly utility allowance.
- The lessor reserves the right to meet Fair Market Rents in May of each year.

By my/our signature(s) below, I/we state that I/we have read and understand the above lease addendum and agree to abide by the terms and conditions herein, and that failure to follow the lease provisions will be just and proper cause for termination of the lease. False, misleading, or incomplete information may result in the termination of the lease agreement.
